NorCal Continuum of Care

NorCal Continuum of Care (CoC) is a consortium of individuals and organizations with the common purpose of planning a housing and services system for people who are homeless. The CoC serves as a convening entity tasked with ending homelessness in the counties of Modoc, Shasta, Lassen, Plumas, Sierra, Siskiyou, and Del Norte. The CoC is responsible for managing Housing & Urban Development funds for homelessness and is uniquely positioned to identify system needs and take steps to address them with the collaboration and partnership of community stakeholders. What does the NorCal Continuum of Care do?

Homeless Management Information System (HMIS):

HMIS is a technology system used to collect client-level data and data on the provision of housing and services to homeless individuals and families at risk of homelessness. For more information and resources for HMIS, please visit norcalunitedway.org/hmis.

Coordinated Entry Process:

The goals for coordinated entry processes are that assistance be allocated as effectively as possible and easily accessible no matter where or how people present.

Continuum of Care Competition:

A comprehensive, collaborative application process that consolidates community-wide information into a single application to HUD for homelessness assistance funding.

Point-in-Time & Housing Inventory Counts:

A count of sheltered and unsheltered homeless persons and housing available to them on a single night during the last ten days of January each year.

Community Partnership:

Our coalition comprises representatives of organizations from nonprofit homeless providers, government stakeholders, public housing agencies, healthcare providers, law enforcement, educational institutions, and formerly homeless and homeless persons. Our planning, coordination, and advocacy alliance develops recommendations for programs and services to fill the need across a seven-county region.
