Housing Supportive Services

The Housing Supportive Services Program helps individuals and families achieve and maintain stable housing by providing personalized support, advocacy, and connections to community resources. Our team works alongside participants to address barriers to housing stability and promote long-term self-sufficiency.

Whether someone is seeking housing, transitioning into permanent housing, or working to remain housed, our staff provides guidance and support every step of the way.

Services

Housing Supportive Services may include:

  • Housing navigation and housing search assistance
  • Assistance completing housing applications and required paperwork
  • Case management and individualized support
  • Referrals to community resources and partner agencies
  • Coordination with landlords, property managers, and service providers
  • Assistance accessing financial assistance programs, when available
  • Life skills and housing stability support
  • Ongoing follow-up to help participants maintain stable housing

Referral Process

Housing Supportive Services is a referral-based program. Individuals are referred through partnering agencies or other approved referral sources. Eligibility and available services may vary depending on the funding source and program requirements.
